REFERRALS

Patients should be referred by a primary care or specialty provider.

  

Referral requests should be accompanied by relevant records that will be reviewed by one of our physicians to determine the appropriateness and urgency of the referral prior to an appointment being made. Appropriate referrals include patients with various forms of arthritis or with systemic rheumatic diseases such as lupus, scleroderma, polymyositis, vasculitis and others. We also evaluate patients with osteoporosis.

All office visits are by appointment only. We make a sincere effort to be on time and expect the same of you. If you cannot keep an appointment, please give us 24 hours notice. If you cancel or do not show for a new patient appointment, we may not make another new patient appointment for you. If you need to cancel a follow-up appointment, we will make an effort to reschedule the appointment as soon as our schedule permits. You will be reminded of your appointment 48 hours in advance by our automated reminder system.

Rheumatology Associates accepts most major health plans, including most HMOs and PPOs.

Billing - Rheumatology Associates' policy is that COPAYS ARE DUE AT THE TIME OF SERVICE. If you are unsure what your copay amount may be, please contact your insurance carrier. The patient is responsible for making sure that we have the most up to date insurance information. Please provide a copy of your current insurance card at each appointment. We ask that our patients understand their insurance company's policies regarding insurance referrals, deductibles and other cost shares, and whether or not we are in your insurance company's network of participating providers prior to your appointment. Your failure to follow your insurance company's policies may result in your being financially responsible for the entire bill.
